● ราคาดังกล่าวรวมค่าแพทย์และค่าบริการโรงพยาบาล
● ในกรณีที่ลูกค้ามีการรับบริการเพิ่มเติมนอกเหนือจากในโปรแกรมดังกล่าว ลูกค้าจะมีค่าใช้จ่ายเพิ่มเติม
● เนื่องจากสินค้าและบริการเป็น โปรแกรมตรวจสุขภาพ จึงไม่สามารถยกเลิกหรือขอคืนเงินเป็นบางส่วนของสินค้าได้
● ไม่สามารถคืนเงินได้ในกรณีที่มาใช้บริการแล้ว
● ไม่สามารถออกใบเสร็จของทางโรงพยาบาลสมิติเวชได้ จนกว่าจะมาเข้ารับบริการ

● สำหรับคำสั่งซื้อก่อนวันที่ 1 มีนาคม 2568 สามารถใช้ได้ถึงวันที่ 31 มีนาคม 2568
● สำหรับคำสั่งซื้อตั้งแต่วันที่ 1 มีนาคม 2568 เป็นต้นไป สามารถใช้ได้ถึงวันที่ 30 มิถุนายน 2568

Remark
● Price includes doctor’s fees and outpatient service fees.
● Services received in addition to those specified in the program are subject to additional costs.
● Products and services constitute one health examination program; cancellation or refund of any individual part of the program is not permissible.
● Coupon is not refundable after redemption.
